CLIENT MATERIALS
The Client Consent Form is the written agreement between Dr. Barnes and each new client about the therapy relationship being initiated. This document includes information about the risks and benefits of treatment, payment, and limits to confidentiality. This document is signed by Dr. Barnes and each new client at the beginning of the first session.
The Notice of Privacy Practices lists in great detail the rules and regulations related to confidentiality and privacy. If you are a current or prospective client please contact Dr. Barnes with any questions.
The Receipt of Services provides brief information about a client's target problems being treated, the treatment they are receiving, and Dr. Barnes' credentials. This can be filled out at each session so that clients can pursue direct reimbursement from their insurance carrier.
